A friend is doing repair on a vintage Jim Phillips board and came across this waveset box. I personally have not seen this rendition. Any info is appreciated.

No it’s a waveset, clearly marked at the bottom of the box.
I would try contacting Kirk Putnam (keeper of all things surf), Bill “Blinky” Hubina at Ventura Surf Shop (one of Tom Morey’ s first employees) or Dennis Ryder (also worked for Morey). Good luck.
